Arizona Mohave County Notice of Claim form

Unlike other counties, Mohave County has NO public info on filing a notice of claim and it does NOT make the form available online.

12/5/22 I contacted the County Attorneys’ Office and was advised to contact Risk and Emergency Management Director, Byron Steward, on this email. Please send an email to Byron, requesting a notice of claim form, and he can email a form to you. Byron’s email is: StewaB@mohave.gov
